Fine Print
- Membership is offered by Raise The Bar Med Spa, PLLC and may be hereby referred to as "The Company"
- The term guest may be used interchangeably with new patient or new client.
- To sign up for a membership, persons must be 18 years old or 13 years old with a parent/guardian signature.
- Your monthly membership option will be billed monthly on the draw date set at the time of the membership enrollment.
- There is an initial minimum enrollment of 6 months. After 6 months the membership becomes month-to-month. If a member cancels prior to completion of 180 days of a new membership contract they will be charged for the remaining unpaid months of their 6 month membership term, as well as for any services provided in the previous 30 days (total charges will be the difference between the non member rate and the member rate paid or compensated- for any services).
- Membership is immediately active upon first payment and signing of this agreement with The Company.
- To cancel the membership: a member must text the office phone at 480-493-5069 and receive a reply at least 30 days prior to the next billing date. If a member is unable to text, please call the office for an email address to send cancellations to 30 days prior to the next billing date. A response by The Company must be received by the member for confirmation of cancellation. If you do not receive a response via text or email from The Company, please call the office at 480-493-5069.
- The Company reserves the right to cancel any membership at any time for any reason and will renew Memberships at its discretion. You may not rejoin membership for 12 months upon cancellation.
- If you commit any of the following violations, it will be grounds for automatic termination of your Membership: (1) you provide false or misleading health history, health status, or other personal information; (2) you provide invalid or improper payment details in any respect; (3) you fail to provide updated payment information after one (1) EFT automatic payment failure within thirty (30) days of failed payment; or (4) you violate any other portion or requirement of these terms and conditions or you violate any other policy of the Company. You will no longer be obligated for future Membership fees and this Agreement will cancel upon the occurrence of the following: (1) you no longer reside within fifty (50) miles of a Company facility; (2) you become physically incapable of (according to a physician’s written medical opinion that can be provided to the Company) traveling to a Company facility or receiving cosmetic medical treatments; or (3) the Company facility closest to your residence permanently closes for business and no other Company facility is located within fifty (50) miles of your residence.
- Moving membership pricing tiers requires a new membership agreement.
- Membership(s) may not be transferred to any other person or entity at any time for any reason. If a member is unable to use their membership option they may speak to management to discuss options for use/future use.
- Referral credits are awarded to current members whose account is in good standing.
- Current members in good standing may use their membership pricing on guests that are new patients. Shared membership pricing is only applicable for guests/new patients to the practice. The guest must have their own appointment and the current member must accompany that guest.
- Current members in good standing are welcome to a $75 retail credit for the month (listed on a government issued identification card) of their birthday that may be used in person only for retail products currently in stock. Credit has no cash value if not redeemed in its entirety.
- Membership benefits do not correspond with special treatment, guaranteed provider availability or access to the business facility outside of business hours.
- If The Company is to change these terms and conditions at any time a notice of the change of terms and conditions will be provided to members within 60 days of the changes. The Company reserves the right to change, add, or remove services and/or products available to members or other clients.
- Payments. All payments are due on the draw date set at time of membership agreement signing. All payments will be collected by electronic funds transfer (EFT) and charged to the valid debit or credit card provided by the member, which will be kept on file with The Company. The member is responsible for ensuring that the provided debit or credit card on file remains active and maintains the proper funding to cover the membership fees. If EFT payment is declined or not honored for the monthly membership charge, The Company reserves the right to demand unpaid membership dues and/or the remaining membership dues from membership term in full.
